Thanks so much for visiting our site. We look forward to meeting you and helping you with your medical issues that have brought you to our practice. Dr. Danziger will meet with you personally to get a detailed history of your condition. He will then perform a detailed physical exam (with a nurse present). He will explain his medical diagnosis and treatment plan and he will be very happy to answer all of your questions and address all of your concerns during the new patient visit and follow-up visits as well.
We highly suggest that you contact your health insurance company prior to your new patient visit to check your insurance benefits. If you wish you can take a picture (front and back) of your insurance card and email it to us (office@nozdoc.com). Please note that most insurances have co-pays or deductibles. Please be prepared to cover these amounts at the time of service.
Please be sure to bring your insurance information and identification to the new patient visit. We regret to say that if this information is not available we will need to reschedule your appointment or we will offer for you to pay for your visit. If you do not have health insurance we accept all major credit cards. If you have questions about your health insurance please feel free to call our office manager Ms. Patterson at 941-761-1911. Please click on the "NEW PATIENT FORMS" tab below to find our new patient forms. We would greatly appreciate that you complete these forms before you arrive for your new patient visit.
